Pleural Effusion (Fluid Around the Lung)
Causes, Symptoms and Modern Treatment

What is Pleural Effusion?
Pleural effusion is a medical condition characterized by the accumulation of fluid in the space between the two layers of the pleura surrounding the lungs.
This fluid buildup puts pressure on the lung, which may lead to shortness of breath and other symptoms depending on the amount and cause of the fluid.
Causes of Pleural Effusion
Pleural effusion can occur due to a variety of causes, including:
Heart diseases
Such as congestive heart failure, which increases pressure in blood vessels and leads to fluid leakage.
Lung infections
Such as pneumonia or bacterial infections.
Liver diseases
Such as liver cirrhosis, where fluid may move from the abdomen into the chest.
Cancer
Including lung cancer or metastasis to the pleura.
Trauma
Such as chest injuries or accidents.
Kidney failure
Which leads to fluid retention in the body.
Symptoms of Pleural Effusion
Symptoms depend on the amount of fluid and may include:
-
Shortness of breath
-
Chest pain (worsens with breathing)
-
Dry cough
-
General fatigue
Diagnosis of Pleural Effusion
Diagnosis is made through:
-
Clinical examination
-
Chest X-ray
-
Ultrasound
-
CT scan
Thoracentesis (Pleural Fluid Aspiration)
This is one of the most important diagnostic procedures, where:
-
A sample of pleural fluid is withdrawn
-
The fluid is analyzed to determine the cause (infectious, malignant, or other)
Treatment of Pleural Effusion
Treatment depends on the underlying cause and severity:
Treating the underlying condition
Such as:
-
Heart failure management
-
Treatment of infections
-
Cancer treatment
Fluid drainage
In moderate or severe cases:
-
Thoracentesis (needle drainage)
-
Or insertion of a chest tube for continuous drainage
Medical treatment
May include:
-
Diuretics
-
Antibiotics
-
Other medications depending on the cause
Surgical treatment
In recurrent or complicated cases:
-
Thoracoscopy (VATS)
-
Decortication
-
Pleurodesis to prevent recurrence
